REGISTRATION FORMS
Pop Warner Football and Cheer require participants to complete nationally
mandated forms to be eligible for games and competition. No other forms may
be substituted.

TO MAKE YOUR REGISTRATION EXPERIENCE MORE EFFICIENT, PLEASE
DOWNLOAD THE FORMS BELOW AND BRING THE COMPLETED FORMS TO
REGISTRATION.

PHYSICAL FORMS MUST BE COMPLETED BY A QUALIFIED MEDICAL
PROFESSIONAL.

THE PARTICIPANT CONTRACT, PHYSICAL FORM, ADULT BEHAVIOR POLICY,
BIRTH CERTIFICATE AND REPORT CARD MUST BE SUBMITTED PRIOR TO
RECEIVING UNIFORMS AND EQUIPMENT. PARTICIPANTS WILL NOT BE
ISSUED UNIFORMS AND EQUIPMENT UNTIL ALL FORMS ARE SUBMITTED.

VOLUNTEERS
For the 8th consecutive season, Pop Warner programs nationwide will be
required to annually conduct a background check of: Coaches, Board of
Directors members and any other persons, volunteers or hired workers, who
provide regular service to the league and/or have repetitive access to, or contact
with, players and/or spirit participants. The purpose of these background checks
is, first and foremost, to protect the children.

Board members, coaches, and volunteers must complete a Volunteer
Application, with a copy of Drivers License/State ID Card, and submit prior to the
